Our History reflects years of experience
With over 33 years of delivering high quality primary care family medical services at its current location, the medical staff at Forest Hill Family Health Associates, PA has humble roots. Established by a Board Certified Family Physician in 1984 and still under his direction, Thomas Ortiz, MD, FAAFP established this practice in an underserved area of Newark NJ, in fulfillment of his obligation to the National Health Service Corp. for their support of his medical school education while at New Jersey Medical School. Dr. Ortiz dedicated his career to the medical treatment of the poor and underserved community. And the community responded with its support of the new practice and the family doctor. FHFHA has been a cutting edge primary care practice with innovation and quality care ever since. With several generations of family members attending the medical staff, high quality outcomes have been achieved. An early adopter of electronic medical records for better patient care the practice has become nationally recognized by NCQA as a Patient Centered Medical Home.
